Short Term Case Management services are designed to quickly respond to individuals requiring immediate supports to prevent or resolve a crisis situation. Services focus on system navigation by connecting individuals with community resources and other service providers, in addition to providing any other supports required to mitigate the immediate crisis situation. Supports might include completing applications, making referrals and providing education, support, and training to the individual and their support network; including family, caregivers and other service providers. Services aim to support overall stability, safety and security within a short period of time, generally less than 6 weeks.
